You would be joining a very friendly, supportive multi-disciplinary team, to help with demands of same day appointments, as well as the ongoing requirements of general practice. St Michael’s is a training practice with 8,300 patients and are based across two sites on the south side of beautiful Bath. We are proud members of Minerva PCN.
As an Advanced Practitioner you will exercise advanced clinical expertise, acting within your professional boundaries, providing advanced clinical skills and advice.
If you are passionate about improving patients’ health outcomes through the demonstration of the sound understanding of the issues related to the identification, assessment, diagnosis, treatment, and management of patients within your care; as well as working collaboratively with the multidisciplinary surgery team this maybe the job for you.
Main duties of the job
As an Advanced Nurse Practitioner, you will deliver high-quality, patient centred care while working autonomously within your professional scope. You will assess, diagnose, and manage patients, making clinical decisions in line with established procedures and protocols set out by our GPs.

- Diagnosing and treating patients independently
- Prescribing medications safely and appropriately
- Requesting and interpreting investigations
- Delivering evidence-based interventions to optimise patient outcomes
- Working collaboratively with our multidisciplinary team to ensure excellent continuity of care

Job responsibilities
Clinical
- Receive patients with undifferentiated and undiagnosed problems and make an assessment of their healthcare needs
- Diagnose and manage presenting conditions using advanced clinical skills, critical thinking, and evidence-based practice.
- Provide urgent, same day, and routine appointments for patients of all ages.
- Triage patient queries and manage clinical workload efficiently.
- Provide health promotion, disease prevention, and lifestyle advice.
- Assess, manage, and treat minor injuries and illnesses.
- Safeguard children and vulnerable adults, following local and national policies.
- Prescribe medications and treatments independently within professional competencies.
- Assessment, diagnosis, treatment, prescribing, referral and discharge as appropriate and deliver key elements of NSFs, providing public health, health protection and promotion programmes that improve health and reduce inequalities
Administrative Responsibilities
- Maintain accurate, legible, and contemporaneous patient records.
- Support delivery of QOF, and other contractual requirements.
- To collect data and participate in regular Practice audit
- Contribute to the development of practice policies and service improvements.
Other
- Work closely with GPs, nurses, pharmacists, social prescribers, and external services to ensure integrated patient care.
- Liaise with secondary care, community teams, and allied health professionals as needed.
- Participate in MDT meetings, case discussions, and safeguarding forums.
- Support the development of protocols, pathways, and best practice guidelines.
- To maintain a safe and therapeutic environment for all Primary Health Care Team staff, patients, carers and co-workers from other agencies. Ensure that all statutory regulations e.g. Health & Safety, COSHH, etc are implemented and adhered to.
- To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice
- To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice

Person Specification
Experience
Essential
- Evidence of working autonomously and as part of a team.
- Experience of use of a medical software package (ideally TPP SystmOne, but training can be provided on this)
- Understanding of evidence-based practice.
- Proven ability to evaluate the safety and effectiveness of own clinical practice.
- Current driving licence (as the role is based across the two sites)
Desirable
- Experience of working within a Primary Care setting.
- Experience in management long term conditions e.g. asthma, COPD, diabetes, CHD.
Qualifications
Essential
- Registered General Nurse/Paramedic (Currently registered with the NMC or HCPC) with extensive post registration experience.
- Masters level qualification or equivalent in advanced clinical practice.
- Independent Non Medical prescriber.
Desirable
- Current IRMER training and competency in requesting appropriate radiological investigations
