Social prescribing

Wellbeing is holistic, and a ‘social prescription’ offers the kind of remedy that doesn’t come in a tube or a bottle. Our PCN’s social prescriber Rachel Dawson has time to unpick a patient’s underlying needs and address complex barriers enabling them to take charge of their own health and wellbeing.

Rachel will listen and help you identify your needs, giving time to explore what matters most, through conversations and information. connecting you with individual and community solutions to help such as

This wellbeing conversation may touch on any aspects of life affecting a person's health and wellbeing, such as:

- Low mood or anxiety

- Low confidence

- Lifestyle  (think sleep, substances, diet and exercise) 

- Carer strain

- Isolation

- Financial stressors

- Impact of long-term health conditions

- Complex social needs

- Low mood or anxiety

- Low confidence

- Looking after yourself (the daily skills of life) 

- Work, volunteering and other activities (how you spend your time) 

- Housing and neighbour issues

- Relationships  

 

Example patient questions which Social Prescribing might help with:

What can I do about my condition?

How can I make the changes I want, to manage my pain?

What hope is there for my situation? (bereavement, neighbour issue, financial hardship, relationship breakdown, workplace stress, carer strain)

Who can help me build confidence to go out more?

Would I be eligible for any help for my care and support needs?

I want to lose weight, stop smoking, reduce my addictive behaviours. Where do I start?

What is going on locally to help me exercise more?

I think I need to talk to someone.

Social prescribing is offered at your surgery on the days above with a Rachel Dawson

There are three appointment types

  • an initial 20 minute telephone triage
  • 30 or 45 minute follow up appointments according to needs

Appointments can be made by all staff

Patients can self-refer via reception

There are lots of local groups, services and activities that you can get involved in. Find out what's going on in your community.

Live Well

Live well B&NES Bath & North East Somerset's community website provides information, support and signposting for families, children and young people with additional needs and adults needing care or support.

Available 24/7 (online) Live Well B&NES supports:

  • Adults needing care and support to live independently
  • Carers looking for support for themselves and their loved ones
  • Parent Carers of young people with Special Educational Needs and Disabilities
  • Young People with additional needs
  • Young People looking for universal information to promote their mental health, opportunities and engagement
  • Parents, children and families, including parenting support and those searching for Ofsted registered childcare within B&NES

Live Well B&NES signposts to a range of services that offer support to our local communities, those provided by charities and voluntary groups, local community enterprises and businesses, as well as services provided by B&NES Council and NHS services.

Wellbeing Courses

The Wellbeing Courses website is a one stop shop for FREE community courses with online and face to face courses, some delivered directly by the Wellbeing College and some by 3rd sector colleagues.
